10/19/2005 Ballot printing CO Sequoia Arapahoe County. Voters in at least four precincts were mistakenly sent duplicate ballots for the Nov. 1 election. The duplicate mailings are a result of an error on the part of Sequoia Voting Systems, a vendor that the county contracts with to print and mail out ballots. Story
12/12/2004 Canvass anomalies NV Sequoia There were about 10,000 undervotes in the state's two most populous counties, Clark and Washoe -- ballots that had no vote for president. Story

11/2/2004 Machine malfunction LA Sequoia State election officials received about 200 complaints of problems with machines, including two confirmed reports of Sequoia AVC Advantage voting machines in New Orleans Parish that were not working. Story Archive
11/2/2004 Machine malfunction NM Sequoia New Mexico Republicans complained at Bush-Cheney headquarters in Albuquerque that Sandoval County e-voting machines switched their Bush votes to Kerry. One Republican candidate for judge found he couldn't vote for himself at first. Story Archive
11/2/2004 Machine malfunction WA Sequoia Snohomish County. Voters in at least four polling precincts in Snohomish County said they have encountered problems with the Sequoia electronic voting machines. When they touched the screen to vote for a candidate, an indicator showed they had selected the opposing candidate. It took at least four attempts before the indicator showed the correct candidate. 10/30/2004 Machine malfunction FL Sequoia Palm Beach. After a woman finished voting, she realized the touch-screen hadn?t given her the option to vote on the two referendums for Boca Raton or for state House District 87. She was given the wrong ballot because the computer was programmed for the wrong ballot, but she can't re-vote. Story Archive
10/26/2004 Machine malfunction NM Sequoia Sandoval County problems escalate. Sequoia machines switch votes, won't register votes unless certain other races are voted, and switch straight party votes to another party. A check of the early voting site Friday morning found that problems were likely due to an "inconsistent stream of electricity," to some of the voting machines. Story Archive

10/20/2004 Poor design FL Sequoia Palm Beach County. Poll workers closed the election on machines, rather than just shutting down for the night. Machines can't be used again in the election.

10/19/2004 Machine malfunction FL Sequoia Palm Beach County. In the second day of early voting, touch screen machines failed and had to be replaced. One machine froze as a voter was voting.
