11/16/2004 Ballot printing FL Escambia County. While the machines accurately counted the number of absentee votes, they began feeding an incorrect result into the office's computers. The tabulation error showed more than 85% turnout, when it was closer to 75%. Story Archive
11/14/2004 Ballot printing CO Instructions on absentee ballots were wrong in several counties, including Denver and Adams. Story Archive
11/9/2004 Ballot printing CO Boulder County. A printing error that distorted bar codes on Hart Intercivic paper ballots is being blamed for delays that made this one of the last counties in the nation to report election results. Scanners rejected ballots with the bad bar codes, requiring election judges to tally those votes race by race. Voting equipment was tested before the election. But the printing error occurred only on actual ballots that went to voters, not the test ballots. Story1 Story2 Archive1
11/1/2004 Ballot printing WA Franklin County. About 25 absentee ballots were mailed without the secrecy sleeve. Voters are asked to place the ballot in a plain white envelope or request a secrecy sleeve from the elections office. Story
10/30/2004 Ballot printing CA San Diego. The first blunder occurred the first week of October, when the county had to send warning letters to 36,000 voters because of a mix-up with absentee ballots at a printing company in Fresno. Wrong ballots were stuffed in some of the ballot envelopes mailed to voters. So far, 65 voters have reported receiving the wrong ballots. Story
10/30/2004 Ballot printing CA San Diego. The method of determining if the absentee ballot is correct doesn't work. One voter reported, "One side was for San Diego, the other side was for Lemon Grove. The front of the ballot is correct. It's the back of the ballot that's wrong. Story
10/30/2004 Ballot printing CA Most absentee ballots had blacked-out portions on the back, the result of elections officials trying to correct a mistake without reprinting ballots. Story Archive
10/30/2004 Ballot printing NE Cass County. About 925 absentee voters had received absentee ballots without the necessary officials signatures. They won't count. New ballots are being sent out to those voters. Story Archive
10/26/2004 Ballot printing FL Palm Beach County. LePore said she received one report of a blank page on a ballot. She attributed that to a printing problem. She said there have been "sporadic" reports of people receiving incomplete ballots, but the problem is not widespread. Story Archive
10/26/2004 Ballot printing NY Monroe County. An Albany judge ruled that the Independence Party's nomination of two state Supreme Court candidates should be disallowed because of problems at the nominating convention. Absentee ballots are incorrect and voting machines must have the candidates changed. Story Archive
10/25/2004 Ballot printing WA Chelan County. Absentee ballots must be reprinted. The auditor did not include the names of two judges, because she thought they didn't need to be on the ballot since they ran unopposed in the primary. Story Archive
10/22/2004 Ballot printing IL DuPage County. A sample ballot printed in 43 newspapers misplaced a Democratic candidate and instructed voters to choose two candidates where only one will be elected. County officials say the actual ballots will be correct, even though the legal notice is incorrect. Story Archive
10/22/2004 Ballot printing IN Marion, Vigo, Clay, Monroe, and Owen Counties. A judge ordered hundreds of absentee ballots to be completely thrown out because one candidate in one race was incorrect.

10/21/2004 Ballot printing ND Cass County. ES&S printed 120,000 ballots with incorrect text. Both "yes" and "no" would approve a constitutional amendment banning gay marriage. Bar codes used for alignment were printed incorrectly, too. ES&S is responsible and will pay for reprinting. Story Archive

10/20/2004 Ballot printing OH Cuyahoga County. Arrows on absentee ballots don't line up with the correct punch hole. "If absentee voters cast their vote by trying to line up the arrow with the punch card, they could punch the wrong number."
